Medicare Advantage vs. Medicare Supplement: What’s the Difference?

Choosing the right Medicare plan is one of the most important decisions you’ll make as you approach retirement. It not only impacts your coverage and care—it also affects your finances. Many people are surprised to learn they have multiple options when they become eligible for Medicare. The most common decision? Choosing between a Medicare Advantage Plan (Part C) or a Medicare Supplement Plan (Medigap). While both help fill gaps in Original Medicare, the way they work is very different.

Understanding the Basics

Original Medicare includes:
Part A (Hospital Insurance)
Part B (Medical Insurance)

Together, these parts cover many—but not all—healthcare costs. You’re still responsible for deductibles, copayments, and coinsurance. That’s where Medicare Advantage and Medicare Supplement plans come in.

Medicare Advantage (Part C)

Medicare Advantage plans are offered by private insurance companies approved by Medicare. These plans:
– Combine Part A and Part B into a single plan
– Often include Part D (prescription drug coverage)
– May offer additional benefits like dental, vision, and wellness programs
– Typically operate within HMO or PPO networks
– Have lower premiums, but higher out-of-pocket costs when you need care

Medicare Supplement (Medigap)

Medicare Supplement plans are also offered by private insurers but are designed to work with Original Medicare, not replace it. Medigap plans:
– Help pay for costs Original Medicare doesn’t cover (like deductibles and coinsurance)
– Do not include Part D—you’ll need a separate prescription drug plan
– Allow you to see any doctor who accepts Medicare nationwide
– Have higher monthly premiums but often result in lower out-of-pocket expenses
– Provide greater predictability and flexibility in care

Did You Know?
You can’t be enrolled in both a Medicare Advantage plan and a Medigap plan at the same time. It’s one or the other.

Key Differences at a Glance

| Feature                  | Medicare Advantage       | Medicare Supplement (Medigap) |
|————————–|————————–|——————————-|
| Includes Drug Coverage   | Often Yes                | No                            |
| Provider Choice          | Network-Based            | Any Medicare Provider         |
| Monthly Premium          | Lower                    | Higher                        |
| Out-of-Pocket Costs      | Higher When Used         | Lower and More Predictable    |
| Extras (Dental, Vision)  | Often Included           | Not Included                  |

How to Choose What’s Right for You

Ask yourself the following:
– Do I travel frequently? If yes, Medigap may offer better flexibility.
– Do I prefer a lower monthly cost or lower costs when I need care?
– Are my doctors in a Medicare Advantage network?
– Do I need extras like dental and vision coverage?

There’s no one-size-fits-all answer. The right plan depends on your medical needs, financial situation, lifestyle, and personal preferences.

Real-Life Example

Let’s say you’re healthy, live in one state year-round, and don’t see specialists regularly. A Medicare Advantage plan with low premiums and some added perks might be a good fit.

But if you have chronic conditions, travel often, or want freedom to choose doctors without network restrictions, a Medigap plan could give you better peace of mind—even with a higher monthly premium.

When to Enroll

Medigap: The best time to enroll is during your Medigap Open Enrollment Period (6 months starting the month you turn 65 and enroll in Part B). After this window, you may be subject to medical underwriting.
Medicare Advantage: You can enroll during your Initial Enrollment Period, the Annual Enrollment Period (Oct 15–Dec 7), or during a Special Enrollment Period if you qualify.
